Pre-Registration Instructions
Covenant Heart and Vascular Institute offers pre-registration services to make your admission process a positive experience.
You will pre-register for your procedure at the Surgical Assessment Services (SAS) office located at in the Covenant Lakeside Medical Office Building located at 4102 24th Street, Suite 100.

Monday through Thursday: 7:00am – 7:00pm
Friday: 7:00am -6:00pm

Surgical Assessment Services’ staff provides friendly service to patients in a comfortable, relaxed setting. Patients scheduled for surgery or a procedure may come to SAS to complete the Patient Health Assessment, consent forms, lab work, EKG and chest X-Ray necessary prior to the procedure. A nurse will review the Patient Health History, obtain a current medication record, review procedure location, and answer any questions.

Arrival Time
Please call (806) 725-1516 the business day prior to the procedure between 3:00 – 5:00 pm for your scheduled arrival time. Times are assigned to help minimize the pre-procedure wait time, but delays can occur. Please be prepared to stay all day.
Living Will
All hospitals are required by law to make available information on living wills or advanced directives to every patient. If you already have one of these, please bring a copy with you. If you have given a copy to SAS, it will be included in your patient information.
Parking
Arrive at the main entrance to Covenant Heart and Vascular Institute on the south side of valet parking. Valet parking will be available starting at 5:30am for a fee of $4 and can be accessed at the 21st Street entrance. You are also welcome to use the east parking garage located directly south of the Heart and Vascular Institute and can be accessed through 21st Street as well. When admitted, you will be given a pass to park in the garage free of charge. Check in at the Short Stay Heart Center, 6th Floor.
If you are using Valet Parking, you will enter the Heart and Vascular Institute building through the sliding glass doors in the building to the left of the drive. Take the elevators to the 6th floor.
If parking in the East Parking Garage, go to the 3rd floor and walk across the sky bridge. Go through the Waiting Room on the 3rd floor to the elevators and take the elevator to the 6th floor.
When you arrive on the unit you will be in the waiting room/registration area. The Admitting Clerk will call for you when ready and complete the admission process. You will then be taken to your room.
